When to Use This Form

How to Use This Form

  4. Optional Contact Information: The form allows for anonymous submissions. If you choose to provide your name, email address, and phone number, these fields are optional. Providing contact information can facilitate follow-up for additional details if necessary.

Third-Party Review and Follow-Up

All submissions are reviewed by a trusted third-party fiduciary, ensuring an unbiased and confidential assessment of your concerns. If you request follow-up, the third party will contact you first to gather more information and then refer the matter to the appropriate party within our organization for further action. Your privacy and confidentiality are our priorities.

If you have provided contact information and requested follow-up, you will be contacted by the third party to discuss the matter further.
